Join us to learn more about an exemplary model in community pharmacy that has advanced chronic care for patients by optimizing medication use to reduce downstream healthcare costs. Topics covered will include chronic care management, quality metrics, and more.

Learning Objectives:

At the completion of this activity, the participant will be able to: 

  • Describe opportunities for community pharmacy to improve chronic care for patients by optimizing medication use.
  • Discuss lessons learned from the collaborative “COMM” model, which produced a successful reduction in downstream costs through improved medication management. 
  • Identify strategies to implement pharmacy practice changes to improve healthcare quality for patients.
